Social Security Benefits

Haigh-Black will file Form SSA-721 which informs Social Security of a death. This will prevent you from having to order a certified copy of the death certificate to send to Social Security.

Veterans Benefits
Burial benefits from the United States Department of Veterans Affairs, National Cemetery Administration include:

  • A gravesite - for casket or urn. The gravesite is actually large enough to fit two caskets or urns;
  • Opening and closing of the grave;
  • Perpetual care;
  • Grave liner which encases the casket in the ground;
  • Government Headstone or Marker;
  • Presidential Memorial Certificate.

Veteran Headstone
If you desire a VA headstone for a grave in a non-national cemetery, we will gladly file an application. Please note that cemeteries may have charges for a base for the headstone and to "set" the stone. The time period from when we submit the application to when the memorial headstone is placed on the grave can take as long as 6 to 8 months depending how backed up the orders are to the VA.

Eligibility
Actual eligibility for burial in a national cemetery is determined by the cemetery at the time of need. Haigh-Black will submit an application for burial along with a copy of the DD214 discharge paper. Approvals can be received without a DD214 but it may take a day or two for the cemetery personnel to research the veterans discharge status and eligibility.

There are two active National Cemeteries in the State of Florida. Although we have coordinated and directed services at Barrancas National Cemetery in Pensacola, Florida National Cemetery in Bushnell is much closer to Volusia County.

Florida National Cemetery is approximately 125 miles from the Daytona Beach and Ormond Beach area. It is also approximately 50 miles south of Ocala or 50 miles north of Tampa.

Directions to Florida National Cemetery
The route that we most often use is: Florida State Road 40 to I-75 I-75 south to exit 62 - the Webster Exit Go right off of the exit The cemetery is about one mile west of I-75

Note: To avoid traffic in Ocala, you can use an alternate route around the city to I-75. This alternative route around Ocala is a bit out of the way but can be considerably faster.

S.R. 40 west to S.R. 326 (1 mile east of Silver Springs, there is a sign that says TO I-75 North. Even though it says I-75 North, you can go south when you reach I-75. Another route that some find faster and may be good for those going to the cemetery from New Smyrna and Port Orange is: I-4 to the East-West Expressway East-West Expressway to the Florida Turnpike Florida Turnpike to State Road 50 State Road 50 to I-75 (approximately 48 miles on a 2-lane highway) The cemetery is one exit north on I-75. National Cemetery in Pensacola is a unique cemetery in that it is located at the home of the Blue Angels. Although it is a good distance from Volusia County, we have coordinated and directed burial services there.

Home and Real Property
Before March 1st of the year following a death, you should take a certified copy of a death certificate to the Tax Assessor's Office in the county or counties where property is located in order to insure proper amendment of records. If you own your own home in Volusia County, you will be entitled to a $500 widow/widowers exemption in addition to the normal $25,000 Homestead Exemption. The local office is located at 250 N. To change titles on automobiles, mobile homes and boats contact the local office of the Department of Motor Vehicles. The office is located at 250 N. Beach Street in Daytona Beach. (386) 254-4610.

Life Insurance
Call the insurance company for a policy in your possession to obtain a claim form. As a rule, the insurance agent will assist you with completing the form, but we would be glad to help you. You will need a certified copy of the death certificate. In addition, you should check the coverage of the home, current or former employers, medical, union, health and accident policies & credit accounts which might have death coverage.

CRIME VICTIM COMPENSATION PROGRAM
The Crime Victim Assistance Program provides advocacy and prompt financial assistance for victims of violent crime thru the Florida Division of Victim Services and Criminal Justice Programs.

What Services Are Offered?
Financial compensation and referral services to eligible persons who have been killed or injured as a result of violent crime. Emergency assistance. Information regarding the appellate court process or the status of appellate cases. Victim service programs and referral to local resources.

Who Qualifies?
Any victim who received personal injury or survivors of someone who was killed as a result of a felony or misdemeanor crime punishable under federal or state law including DUI and hit and run. (All other motor vehicle accidents are excluded.) If.......

  • The crime was reported to law enforcement within 72 hours.
  • The application is made within one year of the date of the crime.
  • The victim has fully cooperated with law enforcement and the State Attorney's office.
  • The victim was not engaged in an unlawful activity.
  • The victim did not contribute to his or her injuries or death.

Who Can Apply?
Any adult victim who has been physically injured as a result of a crime. The parent or guardian of a minor or incompetent victim who has been physically injured as a result of a crime. The spouse, parent, sibling, or adult child of a deceased victim. Any other relative applying on behalf of a deceased victim.

What Information Is Needed to Apply?
The law enforcement crime report Proof of crime-related expenses (medical, dental and/or funeral bills.) Proof of third party payments such as insurance, restitution, or settlements. Proof of the time lost from work due to the crime and a doctor's statement certifying disability.

What Benefits Are Available?

  • Loss of support for persons who were principally dependent on a deceased victim who earned income at the time of the crime.
  • Payment of funeral/burial expenses for a deceased victim.
  • Lost wages for a victim who missed work as a result of the crime.
  • Disability allowance when the victim is disabled as a result of the crime.
  • Reimbursement for medical or dental treatment expenses (including ambulance charges, hospital expenses and doctors fees).
  • Reimbursement for prescriptions, eyeglasses, dentures, or prosthetic devices needed as a result of the crime.
  • Reimbursement for mental health counseling.
